Daily Student Illness and Risk Screening

This screening must be filled out by a parent/guardian EACH MORNING for each child that is entering the building that day.  A screening completed the previous evening will not suffice, as we must ensure that new symptoms did not occur in the morning. The online form must be completed prior to 8:30 am, at which time access to the form is turned “off” for the day.  Below are links to the google form, as well as a paper version that you may print off if you missed the 8:30 am cut-off.

To access Alma, go to https://macsnh.getalma.com/.  Your Username should be YOUR first name, then a dot, then your last name (not your child’s name). If you do not know your password, follow the steps below to get a new one.  If you still have trouble accessing Alma after following these steps, please email Mrs. Williams at swilliams@macsnh.org.
